What to grab when you see it, what only looks good, and what you can
outright abuse — checked against the wiki for Repentance, not Repentance+, because
several of these behave differently on a PS5 than everything written for PC.
S20/20 + Mutant Spider (or The Inner Eye)
Run-winningBeats armour
Repentance rewrote 20/20 to remove the fire-rate penalty of the other multi-shot items while cutting damage to 80%. You fire four or five tears per volley at your unpenalised rate.
Why it wins. Near-doubles DPS from one pickup, and it is the single best damage profile against armour — the maximum number of separate small instances per second is exactly what the reduction formula cannot keep up with.
Watch out. The 80% damage cut is real on a low-damage run. In Rebirth and Afterbirth these overrode each other; only Afterbirth+ onward do they stack.
20/20 · Mutant Spider

A spread of four full-damage Brimstone beams, and "20/20 negates the fire rate penalty of the aforementioned items, which results in enormous damage output." Capped at 16 beams.
Why it wins. Arguably the highest raw DPS in the game, and it stays under the per-hit threshold because each beam ticks nine times per shot rather than landing one big hit.
Watch out. The beams fan out — at close range on a small boss you may only land one or two. Multi-shot does nothing for Brimstone without 20/20 negating the penalty.

Run-winningBeats armour
The floating tear gains Godhead's aura, which in Repentance is a flat 2 damage 30 times a second. Park the tear on the boss and walk away.
Why it wins. Sixty guaranteed DPS that you apply while doing nothing but dodging — and 2-damage instances are so far below every pre-cut threshold that they are never reduced. One of the best Mother and Delirium tools in the game.
Watch out. Repentance made the aura flat rather than a percentage of your damage, so it does not scale — it is a floor, not a multiplier. Repentance also gave the tear homing, which can stick it to a trash mob instead of the boss.

Verbatim from the wiki: "The damage of the tears' aura is not affected by the reduction." Soy Milk multiplies how many auras are on screen; its 0.2× damage penalty never touches them.
Why it wins. One of the very few ways Soy Milk is a pure upgrade, and an enormous count of tiny instances — close to the perfect anti-armour profile.
Watch out. Your actual tear damage is still 0.2×, so the build lives or dies on aura overlap. Soy Milk also shrinks tears, which shrinks the aura hitbox.

Run-winningBeats armour
Tech X wins priority and Brimstone's tears-down is not applied: you fire a Brimstone ring, wider than the laser ring, damaging 15 times a second. Add Technology on top and Repentance restores a further 1.5× damage.
Why it wins. Screen-wide, piercing, infinite range, no aiming, and it keeps ticking as long as the enemy is inside. The best all-round anti-armour package there is.
Watch out. It overrides Ludovico and Tiny Planet completely, and it strips Godhead's aura while keeping only the homing. Walking forward lowers your DPS; backing away raises it.

Run-winningBeats armour
Repentance-only: "Automatically charges and fires fully charged Tech X rings." No charge bar to manage — full-size rings, continuously.
Why it wins. Maximum ring count times fifteen ticks each, with small per-tick numbers. Excellent everywhere, and it removes the one awkward thing about Tech X.
Watch out. Soy Milk's 0.2× still applies to ring damage.

Why it wins. A permanently-on, fully steerable, spectral, piercing damage ring with no charge and no aim requirement.
Watch out. Multi-shot does nothing here, and Tech X, Epic Fetus, Spirit Sword, C Section and Haemolacria all override Ludovico — picking any of them up destroys the build.

The wiki's own strongest endorsement: "Enemies, items and even most bosses will be sucked to the single floating tear… making this combination extremely effective in almost all scenarios."
Why it wins. It is your damage and your entire defence at once — everything gets pinned across the room from you. On a grind where runs die to attrition, that matters more than DPS.
Watch out. No effect on position-locked bosses or scripted phases, and it drags your pickups around too.
The Ludovico Technique
SMom's Knife + Brimstone

Adds a barrage of extra knives behind the main one, each dealing 2× your damage — up from 80% before Repentance. No fire-rate cost.
Why it wins. You keep the held knife's 20-hits-a-second melee and gain a screen-crossing shotgun.
Watch out. Against armoured bosses, hold and stab rather than throw. The thrown knife's 6× spike is exactly the kind of instance that gets pre-cut; the held stab is a dense stream of small hits that is not.

Repentance turned this around completely: "Launched knife will orbit Isaac incredibly fast with a much larger hitbox, dealing consistent damage and covering every side of Isaac."
Why it wins. A 360° melee shield that also deletes whatever touches it — offence and defence in one item pairing.
Watch out. Before Repentance the wiki called this not recommended. If you are reading an old guide, it is wrong. You do lose your ranged option while the knife orbits.
Mom's Knife · Tiny Planet
ATiny Planet + Godhead
StrongBeats armour
Tears with damaging auras orbit you, making a damaging shell.
Why it wins. Same logic as Ludovico + Godhead — flat 2-damage instances thirty times a second are invisible to the pre-cut rule — but it defends you as well.
Watch out. Tiny Planet tears will not hit directly in front of you. Aiming away from what you want to kill is often correct, which takes some undoing.

A Repentance mechanic: from 15.00 fire rate the beam becomes permanent while the key is held, "can change its direction while being fired, and can be aimed in any direction with joystick controls." Damage per tick = damage × fire rate ÷ 5.
Why it wins. A permanent, freely-steerable death laser — and the joystick free-aim is a real advantage on a controller that keyboard players do not get.
Watch out. You are betting entirely on fire-rate-into-damage conversion, because Soy Milk's 0.2× is severe. Rubber Cement sends the beam off at right angles.

ABrimstone + Sacred Heart (or any homing)
StrongArmour: fine
"Will cause the beam to make sharp turns and even loops to hit enemies, which is extremely powerful." Sacred Heart is the largest damage multiplier in the game at ×2.3.
Why it wins. A piercing, spectral beam that cannot miss, multiplied by the biggest multiplier available.
Watch out. Sacred Heart's −0.4 tears compounds with Brimstone's ÷3 fire rate, so uptime drops hard — and on a big damage stat a single tick can cross Mother's ~89 pre-cut threshold. Trade damage for fire rate on the armoured bosses.

AThe Ludovico Technique + Dead Eye

"Damage bonus increases until it hits the max of 2×. Since the controllable tear never hits the ground or wall, the bonus cannot be lost."
Why it wins. A permanent, unlosable ×2 on a build that already cannot miss. Free.
Watch out. Dead Eye does not do this for Tech X — picking it up after Tech X has no effect.
The Ludovico Technique
AIncubus (especially as Lilith)
StrongBeats armour
A second copy of your entire attack. BFFS! pushes its tears to 1.5× your damage, 2× as Lilith. In Repentance, Ludovico gives Incubus a second controllable tear.
Why it wins. Doubles your damage sources without increasing per-hit size — exactly right against armour, and it costs you no fire rate.
Watch out. Repentance cuts Incubus damage by 25% for everyone except Lilith and Tainted Lilith. It also ignores a long list of items entirely, including Epic Fetus, Maw of the Void, Tech.5 and Mom's Eye.

Gives Azazel the full-length beam "and it also increases fire rate since Brimstone's fire rate multiplier is ×1/3 and Azazel's is ×0.267."
Why it wins. Removes his only real weakness and speeds him up, on a character who already has the best starting damage and flight. Repentance cut his base range from 17.75 to 4.5, so this is worth more than it used to be.
Watch out. Tech X on Azazel carries a 35% damage penalty in Repentance unless you also have Brimstone. Ludovico halves his damage on its own, likewise recovered by Brimstone.

APolyphemus + 20/20
StrongArmour: fine
Repentance-only: 20/20 "grants additional +5 flat damage and removes the fire rate penalty, firing 2 tears per shot" while keeping Polyphemus's (Damage+4)×2.
Why it wins. Turns a slow sniper into a full-rate double shot without losing the multiplier.
Watch out. The Inner Eye and Mutant Spider are the trap version — with those you lose the ×2 entirely and get only +5 flat, usually a downgrade. 20/20 is the one you want.

APolyphemus + Rock Bottom

"Allows Isaac to keep his previous fire rate in addition to the massive damage boost."
Why it wins. The only way to make Polyphemus competitive against armour, because it restores the hit count that the item normally trades away.
Watch out. You need Rock Bottom before Polyphemus to lock the fire rate in.

Repentance-only: charge scales the beam "from 25% damage and 50% width at minimum charge to 250% damage and 160% width at maximum charge."
Why it wins. A 2.5× Brimstone on demand, with the option to tap-fire chip damage.
Watch out. Inverted against armour. Max-charge shots are the slow heavy attacks that get pre-cut — on Mother a 250% tick clips the ~89 threshold. Fire more, weaker shots at armoured bosses; the opposite of what you do everywhere else. (Before Repentance this pairing gave no damage at all — old guides are wrong.)

Once the highest number in the game. Repentance cut Ipecac's bonus here from +40 to +2 flat.
Why it is a trap. The worst case in this whole list. A 20×-damage missile is one enormous instance that blows past every pre-cut threshold, and Epic Fetus is named on the wiki's list of explosives that do not bypass armour. It will feel unstoppable through the Depths and then flatline against Mother and Hush. Epic Fetus also overrides Ludovico, Brimstone and Mom's Knife, so it eats better builds.

Why it is a trap. The purest looks-amazing-dies-to-armour build there is. Very few, very large instances — a full-charge tear is wildly over Mother's threshold and gets gutted before the sustained reduction even starts. Great for clearing floors, bad for the boss you actually need.

Why it is a trap. Every instinct says stack multipliers onto Ipecac. They do almost nothing, and you cannot brute-force past the armour floor the way a multiplier build normally would.

Why it is a trap. You trade Godhead's entire aura — a flat 60 DPS — for homing you barely need, plus −0.3 tears and −0.3 shot speed. Do not take Tech X if you already have Godhead and a normal-tear or Ludovico build going.

The classic pre-Repentance combo. Ludovico's tick rate is now capped at 30 a second, reached at a modest tears stat.
Why it is a trap. Most of Soy Milk's ×5.5 is thrown away against the cap while its 0.2× damage applies in full. Usually a net loss now. (The wiki does not state this outright — it follows from the tick cap and the multiplier, so treat it as reasoned rather than quoted.)

Why it is a trap. You are surrounding yourself with a live minefield that goes off the moment you move or something touches it. Host Hat or Pyromaniac is close to mandatory; without explosion immunity this ends runs.

Gnawed Leaf — the statue

"After standing still and not pressing the move or attack buttons for one second, Isaac will turn into an invulnerable statue." Bombs and active items break it; placed bombs are safe if you touch nothing after placing.
How. Enter the room, set your damage source going, then let go of everything for a second and do not touch the stick or the fire button.
- Sprinkler — "will continue to fire when Isaac is invincible… This theoretically allows end-game bosses to be beaten without having to move."
- Daddy Longlegs — clears most rooms and bosses on its own. Cannot hurt The Shell, and ignores enemies far from you.
- Ludovico + Strange Attractor — park the tear first, then never touch fire.
- Contact orbitals (Cube of Meat and friends) — slow, but they are invulnerable whenever you are.
- Tainted Forgotten — the Soul stays invincible as long as you never throw the body. The most degenerate version of this in the game.
- Mom's Toenail — buffed in Repentance to 300 damage every 20 seconds, and it only hurts you when you are not a statue.
Cost. Extremely slow. And see the Hush warning below.

Strange Key — delete the 30-minute Hush clock

A trinket added in Repentance. Holding it "unlocks the passageway to ???, regardless of run duration" — the Blue Womb entrance opens after Mom's Heart whether you are at 25 minutes or 90.
How. Unlocked by defeating Isaac, ???, Satan and The Lamb as Tainted Eve — the Cross, Polaroid, Inverted Cross and Negative marks. That is two runs on a character who needs all four anyway: one Cathedral-side, one Sheol-side.
- It survives being smelted, so a Smelter or Marbles turns it into a passive and it keeps working.
- It does not open Boss Rush — that 20:00 gate is separate.
- Careful with Pandora's Box: using the two together consumes both.
Cost. Two runs up front. Against 29 characters owing Hush, that is the best trade on the board — every later Hush run stops being a race.
Strange Key
Mama Mega! — force the doors open

"Mama Mega! opens the paths to Boss Rush and the ??? even after the time limits expire." It also opens every Secret and Super Secret Room door on the floor.
How. One-time use, no recharge. Use it in the boss room itself — leaving the boss room or the entrance room closes the doors again.
- It opens both gates, so unlike Strange Key it rescues a Boss Rush too.
- Suplex! used as a Berserk! effect ends produces the same explosion and the same door-opening, which is a Repentance addition.
- Bag of Crafting: 8 × Giga Bomb, or 2 Golden Bombs + 6 Bombs.
Cost. You have to already have it when the timer runs out. Mega Bean does the same thing only in Repentance+, so it will not work for you.

Rock Bottom — stat flooring

"Prevents Isaac's attributes from falling below their highest values reached." Stats keep being calculated normally underneath; you just never lose ground.
How. Take it, then farm anything that gives a temporary boost. Book of Belial gives +2 damage that "will stay in effect indefinitely… can then be used once again to stack on top of that new floor." Repeat forever.
- Experimental Pills — the stat down never lands. Horse versions are strictly better. (Reasoned from the two mechanics; the wiki does not spell it out.)
- Razor Blade / Berserk! / Red Stew — Red Stew's +21.6 damage becomes permanent.
- Kidney Stone — permanently gives a ×6 fire rate multiplier after firing.
- VII - The Chariot? — its ×4 fire rate can be locked in.
- Ipecac — the huge damage up without the tears down. Order matters: take Ipecac before Brimstone or Tech X or you only get +2.
Cost. Soy Milk is a trap here. The 0.2× still applies underneath, so every future damage up is poisoned — you would need over 5× just to start climbing again. Take your damage ups first. And never drop Rock Bottom: the lock releases.

Damocles — everything, twice

"As long as the sword hangs above Isaac, all item pedestals are doubled." After you take one real hit it can fall at any time and kills you outright — about 4.4% per minute, 50% survival at 15 minutes.
How. The clock only starts on your first real hit. Self-damage does not count — blood machines, devil beggars, curse rooms, spiked chests, bad trip pills. Nor does damage blocked by a shield.
- A shield character keeps it unarmed indefinitely — Holy Mantle, Blanket, Wooden Cross, Cone Head, Host Hat, Infamy, Metal Plate.
- Genesis removes it entirely, and is one of only two things that can (Butter! is the other). Use Damocles first, then Genesis — you keep the doubles.
- Survive the fall with any active shield, invincibility, or Gnawed Leaf statue form. Holy Mantle does not save you from the sword itself.
- Do not use Panic Button — it arms Damocles just before you get hit, guaranteeing the sword later.
Cost. Chest items are never doubled, so it is worth much less in Chapter 6. Priced items are not doubled either.

How. Swipe pickups to store them; the swipe also opens spiked and mimic chests without hurting you, and buys shop pickups you can afford.

Destroys pedestals; each passive absorbed gives two permanent random stat boosts, kept even if you drop Void. Six-room charge in your version.
How. Use it while holding an item over your head and it still absorbs — so you can buy a Devil or shop passive and eat it immediately, or peek at a blind item before deciding.
- Mega Blast / Mega Mush — use them every 6 charges instead of 12.
- Glass Cannon — fires without ever breaking or hurting you.
- Single-use items bank inside it — Mystery Gift, Diplopia, Damocles, Genesis all wait until your next Void press.
- Book of Virtues — one press spawns the whole wisp set.
Cost. Do not void Book of Virtues as an item (you get stats instead of the effect), or Moving Box, Flip, The Jar, Urn of Souls or Abyss — they give nothing. Car Battery makes single-use items fire on absorption instead of banking.

Once you pick up a second active it becomes passive and every active use spawns wisps. Most deal 2× your damage on contact, and up to 26 can orbit you.
How. Hold the NO! trinket and walk into a Library — Book of Virtues is the only passive in that pool, so it is close to guaranteed.
- Book of Belial wisps are the strongest common ones — 3 HP, 6 damage — and stack under Virtues under your real active.
- Crack the Sky wisps call a beam of light for 12 extra damage when touched.
- Jar of Wisps + Car Battery — triples, then doubles: ×6 wisps per press.
- Whenever you are invulnerable the wisps are too — the direct Gnawed Leaf combo.
Cost. It also turns your first Devil Room into an Angel Room and raises Angel chance 25%, which you may or may not want depending on the mark you are chasing.
Book of Virtues
Sacrificial Altar — familiars into Devil items

"Sacrifices up to two familiars and spawns an item pedestal for each familiar sacrificed", from a curated list — Brimstone, Mom's Knife, Incubus, Abaddon, Maw of the Void, Goat Head, Sacrificial Dagger, Death's Touch, Void, Athame, 7 Seals.
How. Collect junk familiars all run, then cash them in.
- Cambion / Immaculate Conception — familiars can be sacrificed without consuming them, and respawn afterwards.
- Blue flies and spiders count, so Guppy-type builds feed it endlessly.
Cost. It picks two at random, so you cannot protect a familiar you wanted to keep. The old Void + Altar infinite loop was patched out in Repentance — it does not work for you.

Chaos Card — what it actually kills

"The thrown card kills anything in its path." It is thrown in the direction your body faces — walk that way first, or on a controller you will waste it.
How. Blank Card re-uses it (6-room charge in your version). The Battery lets you fire two in a room, which is how multi-phase bosses die.
- Ends the fight outright: Isaac, ???, The Lamb, Satan (even in his first form), Great Gideon — who also drops a crawl space with two items and two chests.
- One phase only: Mega Satan, Hush, Mother, Dogma, Ultra Greed in Greedier.
- Does nothing: Delirium, The Beast, Mother's Shadow, Dark Esau.
Cost. There is one room in the Mines with a guaranteed Chaos Card behind a rock puzzle.

- Knife Pieces and Key Pieces respawn on their floors, so you can collect multiples.
- Live in your version, patched in Repentance+: swapping R Key out for another active as you use it lets you carry a different active into the restarted run.
- The wiki notes that with four crafted R Keys you can earn every completion mark for a character in one run, Greed aside.
Cost. The timer does not reset — Boss Rush and Hush windows do not come back. Item pools do not reset either.

Death Certificate — the actual answer

Spawns one copy of almost every item you have unlocked on the save, on pedestals.
How. Two reliable routes: 8 Bone Hearts in Bag of Crafting, or Meat Cleaver → Bot Fly → Death Certificate with Spindown Dice.
- ? Card mimics it without consuming it.
- Jacob and Esau grab two items at once — Brimstone and Mom's Knife sit close together.
- Genesis then Death Certificate turns the Genesis choice pedestals permanent.
Cost. You can softlock yourself by using it after already taking every item in the area.

Clicker — change character mid-run

Turns you into a random character and removes your last item. 100% removal in your version — the gentler 50% version is Repentance+.
How. Non-tainted can only become non-tainted, and tainted only tainted.
- Completion-mark farming: Tainted Cain can craft what he needs, then Clicker across the roster — runs started as Tainted Cain can bank other characters' marks.
- With Rock Bottom you keep the best stats of every character you pass through.
- Greed donation farming — swapping character resets the machine's jam chance.

Glitched items. The wiki carries a bug tagged for Repentance and specifically for PS4 and PS5: "Glitched items can trigger the effect of Wavy Cap, crashing the game."
- Glitched items can spawn a Challenge Trophy or Big Chest, and touching either instantly ends the run and returns you to the title screen.
- Once Corrupted Data is unlocked you can meet glitched items involuntarily in Secret and I AM ERROR rooms.
Cost. Do not pick this up on a run you care about. If you are exposed: Genesis removes the effect, Missing No. rerolls it, Spindown Dice deletes glitched pedestals.
